The day of your visit:
To ensure your best quality care, your medical and surgical history is very important. It is helpful to bring in a list of current medications including doses. This information can be filled out in advance on the health history form which can be downloaded above. In addition, it would be helpful to have copies of any x-rays, CT scans, or MRIs that are related to the problem for which you are being seen. The name and contact information of your primary doctor (as well as any necessary referrals) allows for more efficient exchange of information to promote your thorough, comprehensive care.
